loose shift knob

I was driving yesterday and when I shifted the knob suddenly became loose and started turning clockwise and now it just turns and turns and does not come off nor tightens. Also noticed the shift boot is detached from the knob. Did this happen to anyone?

Yes mine was loose too when I took it out to swap the knob. It looked like it was not glued properly. Bring it back to the dealer and have them fix it.
